Description
About the Service
We are recruiting on behalf of a community based outreach service supporting adults with complex needs including homelessness mental health challenges substance misuse offending histories and social isolation. The service delivers assertive outreach intensive support and crisis intervention in the community aiming to stabilise risk improve wellbeing and enable long term independence. The operating model focuses on trauma informed practice relationship building and multi agency partnership work.
The Role
• Carry out assertive outreach across the community including street based engagement welfare checks and home visits
• Build consistent trust with individuals who may be reluctant to engage with services
• Complete assessments risk planning and outcome focused support plans
• Support individuals to access housing health services benefits treatment programmes and community resources
• Respond to crisis situations escalating safeguarding concerns and coordinating emergency interventions
• Work collaboratively with statutory and voluntary partners including police housing mental health teams and drug and alcohol services
• Assist service users to build life skills structure and routines that promote long term stability
• Maintain accurate records case notes and activity logs in line with service compliance requirements
Essential Requirements
• Experience working with adults who have complex needs and face multiple barriers to engagement
• Strong risk management and safeguarding competence
• Ability to engage individuals in non traditional environments and work independently in the community
• Trauma informed and resilience based practice knowledge
• Skilled communicator with strong professional boundaries
• Ability to manage a dynamic caseload and prioritise effectively
• Full driving licence preferred depending on service model
• Full right to work in the United Kingdom
